PRE-REGISTER

You can download forms off this page, or pick them up in person near the Security Gate at the Sports and Fitness Center of the

Lawrence Family Jewish Community Center (LFJCC)
4126 Executive Drive, La Jolla 92037

Please remember that all Registration Forms and checks are mailed to:

The Lawrence Family Jewish Community Center should not be asked to accept anything at all for Krav Maga San Diego. Students who attempt to register by bringing registration forms/payment directly to the JCC, or mailing them to the JCC instead of mailing them to Krav Maga San Diego, will not be registered for the training and their place will be given to a waiting student.

KMSD REGISTRATION FORMS

Continuing Students, you DO NOT need to fill out a new form for each regular session, unless something changes, i.e. email address, or if you have not trained with us in three or more sessions. All participants in Seminars must fill out a Registration Form for that particular Seminar.
